Medical record management is a crucial component of healthcare services as it provides accurate, complete, and timely patient information for healthcare professionals. Advances in information technology have driven digital transformation in healthcare through the implementation of Electronic Medical Records (EMR), aiming to improve efficiency, data accuracy, and service quality. However, the implementation of medical record digitalization in Indonesian healthcare facilities still faces various challenges, including infrastructure limitations, human resource readiness, and internal policy constraints. This study aims to evaluate and optimize the medical data digitalization process using the Plan–Do–Check–Action (PDCA) method at RS TNI AD Bhirawa Bhakti Hospital. A qualitative descriptive approach was employed through Clinical Field Practice activities, encompassing planning, implementation, evaluation, and follow-up stages. Data were collected through direct observation, discussions, documentation, and process evaluation, and analyzed descriptively by linking quality management concepts with previous studies. The findings indicate that the PDCA approach effectively improves medical data management through the development of standard operating procedures, digital system flow design, electronic form adjustment, and continuous monitoring and evaluation. The implementation of PDCA contributes positively to work efficiency, process consistency, and the quality of medical record management, supporting the sustainability of digital transformation in hospital services.
