Tuberculosis (TB) remains a major global public health problem, with a substantial burden among the productive-age population. Workers represent a high-risk group for pulmonary TB due to exposure to unhealthy working environments, including poor ventilation, overcrowding, and hazardous substances, as well as limitations in the implementation of occupational health policies. Therefore, TB control among workers needs to be examined not only from a clinical perspective but also through a health policy lens.
Objective: This literature review aims to synthesize scientific evidence on pulmonary tuberculosis among workers from a health policy perspective, focusing on disease burden, occupational risk factors, existing health and occupational safety policies, and challenges in policy implementation.
Methods: This study employed a literature review design conducted in accordance with the PRISMA 2020 and PRISMA-S guidelines. Literature searches were performed in major electronic databases and policy repositories using keywords related to tuberculosis, workers, occupational health, workplace, and health policy. Eligible publications included peer-reviewed articles, reviews, and policy documents published within the last five years (≥2021), available in full text, and addressing tuberculosis among workers and related policy implications. Data were synthesized using a structured narrative approach.
Results: The review findings indicate that pulmonary tuberculosis among workers is influenced by a combination of individual factors, workplace environmental conditions, and the strength of occupational health policies. Certain occupational groups, such as healthcare workers and mining workers, exhibit a higher risk of TB. Policies integrating workplace-based TB screening, infection control measures, and social protection are essential for reducing TB risk among workers; however, implementation remains inconsistent, particularly in the informal sector and in monitoring and evaluation mechanisms.
Conclusion: Pulmonary tuberculosis among workers is a multidimensional public health issue that requires comprehensive, inclusive, and evidence-based occupational health policies. Strengthening the integration of national TB programs with occupational health and safety policies, alongside improved policy implementation, is crucial to achieving sustainable TB control among the working population.
