Cold Chain Product (CCP), particularly vaccines, requires proper storage to maintain its quality, safety, and effectiveness. This study aimed to evaluate vaccine storage practices at Pekanbaru Kota Public Health Center based on cold chain requirements. This study used a descriptive observational design with an evaluative approach. Data were collected through direct observation of vaccine storage practices and comparison with applicable guidelines. The evaluated aspects included storage temperature, vaccine refrigerator, vaccine arrangement, First Expired First Out (FEFO) implementation, Vaccine Vial Monitor (VVM) examination, temperature monitoring, refrigerator maintenance, and procedures for handling power interruptions. The results showed that all observed aspects had been implemented at Pekanbaru Kota Public Health Center. Vaccines were stored at 2–8°C using a dedicated vaccine refrigerator in good condition. Vaccine arrangement considered temperature sensitivity, while FEFO and VVM examination were routinely applied. Temperature monitoring was conducted in the morning and afternoon, and refrigerator maintenance and procedures for power interruptions were available. Overall, vaccine cold chain storage practices met the observed requirements. However, further evaluation is needed regarding thermometer calibration, temperature deviation documentation, and corrective actions.
