In Digital Radiography (DR) examinations of the wrist joint using the PA projection, image quality can be compromised by the presence of noise, which reduces the clarity of anatomical details. Research comparing the image quality of PA-projection wrist joint DR examinations performed using a bucky table versus a bucky stand remains limited. This study aimed to evaluate the image quality of PA-projection wrist joint radiographs obtained using bucky tables and bucky stands—based on Signal-to-Noise Ratio (SNR) and Contrast-to-Noise Ratio (CNR) values—and to identify the factors contributing to noise. This mixed-methods study was conducted at Dr. Gunawan Mangunkusumo Regional General Hospital (RSUD) between March and April 2026. Data were collected through observation, interviews, documentation review, and Region of Interest (ROI) measurements using the RadiAnt DICOM Viewer application. Observational, interview, and documentation data were analyzed descriptively, while ROI data were analyzed based on noise levels, SNR, and CNR to objectively assess image quality. The study found that using a bucky stand resulted in a lower noise value (362.81) compared to the bucky table (956.42). SNR and CNR values were also higher for the bucky stand (113.23 and 62.42, respectively) than for the bucky table (60.35 and 26.45). Factors contributing to noise in PA-projection wrist joint DR images taken on the bucky table included the use of lower exposure factors (42.1 kV and 2.25 mAs), the condition of the radiographic equipment, and the digital image processing workflow. The study concludes that using a bucky stand for PA-projection wrist joint DR examinations yields superior image quality compared to using a bucky table, as evidenced by the noise, SNR, and CNR values. The use of a bucky stand may be considered to achieve optimal image quality, and radiographers need to pay attention to exposure factor settings to minimize noise.
